We do ask that you arrive several minutes before your scheduled appointment to complete the needed paper work and allow us to process this efficiently. You may download the Registration, Health History, and Privacy Policy to complete and bring with you.
A thorough health history is very important to your care. Not everyone knows that the links between your periodontal (gum) health and the health of your body are very strong. Gum disease may cause problems with pregnancy, your arteries, your heart and your blood sugar levels. Certain medicines that you take may affect your oral health and treatment options. So, we need you to accurately complete the health history and have an accurate list of any prescription and supplemental medications you take.
If you have records or recent radiographs, please have them with you. We do not require that you do so, as your current oral health status is our main interest; however, in doing an exam we need to be as thorough and normal dental protocol for diagnosis will be accomplished regardless of what has been done in a previous office. We know you desire what is best for you; thus, we do to.
Also, if you are covered by third party plans please have all the pertinent information and your insurance card so that we may see it and check on any needed information.
Normally at this time we will be accomplishing only preventive and diagnostic procedures so that we can give you an idea of your state of oral health. We will then appoint you for the necessary treatment at times that are convenient to you. “Filling Station” dentistry does little for the health of your mouth, and is something to be avoided; so if you want ‘one stop’ dental care, we are not your office. We care about your health.
